DWC Has Revised DWC-3 Wage Statement Forms
On July 27, 2022, the Texas Department of Insurance, Division of Workers’ Compensation (DWC) announced the adoption of revisions to three forms:

- DWC Form-003, Employer’s Wage Statement (DWC Form-003);
- DWC Form-003ME, Employee’s Multiple Employment Wage Statement (DWC Form-003ME); and
- DWC Form-003SD, Employer’s Wage Statement for School Districts (DWC Form-003SD).

DWC further advised that stakeholders “may begin using these forms July 27, 2022.” Implementation is not yet mandatory, however. Based on informal conversations that FOL has had with DWC, we recommend that stakeholders make the transition to the new forms within 90 days.
DWC explained that the adoption of these forms is necessary because:

- DWC Form-003 allows employers a way to provide wage information to the insurance carrier to calculate the average weekly wage and issue income benefits to injured employees or their beneficiaries.
- DWC Form-003ME allows injured employees a way to provide wage information for other employers to the insurance carrier to calculate the average weekly wage and issue income benefits when the injury affects their ability to work.
- DWC Form-003SD allows employers a way to provide wage information to the insurance carrier to calculate the average weekly wage and issue income benefits to injured employees that work for school district.
