DWC Has Revised the SIBs Application (DWC-52)
DWC has revised DWC Form-052, Supplemental income benefits (SIBs) application, effective June 30, 2024.
The DWC Form-052 is necessary for injured employees to apply for SIBs. According to DWC, the revisions were made “to make the form more user-friendly and to clarify work search requirements.”
The form is on the TDI website and can be used immediately. The previous version of the form will be accepted until August 31, 2024. The new form can be accessed at https://www.tdi.texas.gov/forms/dwc/dwc052sibs.pdf.
Notably, the new form expands and provides more detail in the section regarding how the claimant intends to establish an “active work search effort” during each week of the applicable qualifying period, in Part 3 at the bottom of p.1 of the new form.
The section in which to list wages earned during the qualifying period is also expanded, in order to provide more detail, as is the work search log.
