DWC Seeks Comment on Travel Reimbursement Form Revisions
The Division of Workers’ Compensation is accepting public comment on revisions to DWC Form-048, Request for Travel Reimbursement. Tracey Beaver, Director, Office of Workers’ Compensation Counsel, for the Division announced the planned revision in a May 18, 2015 memo. The revisions update the format and style for consistency with other DWC forms. The proposed DWC Form-048 will also contain answers to frequently asked questions to provide additional information about travel reimbursement.
The proposed revisions to the form are currently available here. The public comment period closes June 1, 2015 at 5 p.m. CST. Public comments should be submitted to:
